Mohs micrographic surgery (MMS) is the gold standard treatment for high-risk non-melanoma skin cancers particularly basal cell carcinoma and squamous cell carcinoma. Besides offering superior cure rates, tissue-sparing benefits are useful for cosmetically sensitive areas. I was fortunate to be offered an opportunity to pursue my fellowship training in St John’s Institute of Dermatology at Guy’s Hospital, London, United Kingdom under the supervision of Dr Raj Mallipeddi Dr Mallipeddi is a consultant dermatologist and dermatological surgeon who is the lead clinician at the Dermatological Surgery and Laser Unit (DSLU) at St John’s Institute of Dermatology.


I spent a year in London from May 2021 to April 2022 learning and refining various techniques in Mohs micrographic surgery, dermatological surgery and nail biopsy. As part of the fellowship requirements, I was able to participate in and perform more than 500 cases of Mohs micrographic surgery and more than 400 cases of reconstructions including flaps and full-thickness skin grafts. During the fellowship, there was also exposure to laser procedures, photodynamic therapy as well as botulinum toxin injections for axillary hyperhidrosis and cosmetic indications. Furthermore, I had the opportunity to be involved in various educational and research activities including participation in multidisciplinary skin cancer meetings.


The fellowship was a truly educational and enlightening experience. I particularly cherish the friendships and working relationships forged during my stint. Moving forward, I hope to continue developing the Mohs micrographic service in Malaysia as well as contribute to educational efforts in dermatological surgery locally.
